I work in a psych hospital so we use "advanced directives" and "wills" if a patient has or supplies them. However, these "wills" are more for medical treatment, not psych, wherin an incapacitated pt usually has a guardian with power of attorney to sign all that stuff.

The POLST or Physician Orders for Life-Sustaining Treatment is more immediate betw patient and doctor or health care worker, and is transferable to other facitilities at the request of the patient.

The POLST form facilitates the process of translating end-of-life discussions with patients into actual treatment decisions, and provides security for the individual and physician that the expressed wishes will be carried out. There is no other form that streamlines the process in this way.
